This is mostly allegorical and is not about the type of love that is commonly romanticized in typical greeting cards. It is about the kind of love that goes beyond chocolates, roses, candy, night clubs, parties, rainbows and butterflies. It's about the raw, gritty bits that are found in deep love fostered over years of shared trials, hardships, and challenges. It's about hugs and kisses as expressions of respect and appreciation for trust that has been earned and maintained by making tough decisions, putting in the work, and keeping confidences. It's about the real life "better and worse" kind of love mentioned in many traditional marriage vows. It's about the kind of love that brings two people from different families together into a single larger joined family.

A closer look reveals subtle details not immediately apparent such as drops of blood coming from the thorn of the rose and background with tears incorporated along with numerous "O"s (for hugs) and "X"s (for kisses of course).

If you have known true love, you will understand. If this image makes no sense to you, it is not something that can be explained.

As a final thought, it is surprising to me the number of people who have told me they did not notice the silhouetted couple in the center of the mended hearts.
